Initial Visit
Your first appointment will consist of a comprehensive examination including an oral cancer screening and any necessary digital X-Rays. Upon completion of examination, a consultation explaining diagnosis and treatment options will be reviewed with you.
Please assist us by providing the following information at the time of your consultation:
- Please complete the patient intake form provided on our website. Alternatively, you can print the PDF version here and email or fax it into our office
- A list of medications you are presently taking.
- Photo ID & Insurance card

We understand and accommodate dental emergencies. In case of Emergency please contact our emergency line and leave a detailed message. Our secretary will return the call as soon as possible to set up an appointment.
